Death Resolution Procedure
To obtain an official University Death
Resolution,
please complete the following steps:
1. Complete the Death
Notice/Resolution Request Form. All required fields must be
completed to ensure issuance of the Resolution.
2. Please advise the Office of the Secretary if you would like to pick up
the resolution or whether it should be forwarded to the next-of-kin
indicated on the form.
3. Forward the completed Death Notice/Resolution Request Form via
facsimile to:

Board of Trustees and the President of the University to express the
sympathy of the Board of Trustees on the passing of the deceased and
to note this expression as a permanent part of the University’s historical
record. The Office of the Secretary is committed to fulfilling this
