Death Resolution Procedure

Death Resolutions are generated upon the authority of the Chairman of the Board of Trustees and the President of the University to express the sympathy of the Board of Trustees on the passing of the Deceased and to note this expression as a permanent part of Howard University’s historical record. The Office of the Secretary is committed to fulfilling this expression of goodwill.To obtain an official Howard University Death Resolution, please:

  1. Complete the Death Notice/Resolution Request Form. All required fields must be completed to ensure issuance of the Resolution.
     
  2. Send the completed Death Notice/Resolution Request Form via facsimile to (202) 332-2961.

  3. Please call (202) 806-2250 to ensure that the fascimile has been received and to advise if you would like to pick up the Resolution or whether it should be mailed to the next-of-kin indicated on the form.
