This form is used to request that Information Systems and Services ("ISAS") provide web hosting for your Howard University student organization. Information about how to qualify and the services offered are available at http://www.howard.edu/web/hosting/

Print the form that is generated.
Have the student organization president and the director of Office of Student Activities in Blackburn Center Suite 117 sign at the bottom of the printed form.
Deliver the form to ISAS in Wonder Plaza, 2301 Georgia Avenue, Suite 334, Attention: Andre Mekkawi University Webmaster.
